Receiving a complex neurological diagnosis — a cerebral aneurysm, an arteriovenous malformation, a tumor — usually comes with a difficult decision and little time to understand it. Seeking a second opinion is not a matter of distrusting your doctor: it is a recognized practice in medicine and a patient's right.
Here, a second opinion is a review of the material you already have — imaging studies, reports and the letter from your attending physician — to reassess the picture and discuss with you the possible courses of action, their criteria and what usually weighs on each choice.
Situations in which a second opinion usually makes a difference
You have been diagnosed with a cerebral aneurysm, arteriovenous malformation, cavernoma or brain tumor and surgery has been indicated.
You were advised only to monitor a lesion found on an exam and want to understand the criteria behind that approach.
You received different recommendations from two professionals and need to compare the reasoning behind each.
You live with symptoms — such as trigeminal neuralgia or hemifacial spasm — that still have no defined course of action.
You are a relative or caregiver and are gathering information to decide together with the patient.
Exams and documents for the analysis
Imaging exams
Magnetic resonance imaging, MR angiography, CT, CT angiography or digital angiography (catheterization). The ideal is the original DICOM file — the exam CD, the link from the radiology service or the full folder. That is where the information the report summarizes actually lives.

The PDF report for each exam you send, or a legible photo of the printed document — including the date it was performed and the service where it was done.
Attending physician's letter
The document with the diagnostic hypothesis and the course of action proposed by the professional who follows your case. It is what makes it possible to compare the readings rather than simply repeat them.
Clinical history
When the symptoms began, how they evolved, previous surgeries and hospitalizations, conditions under follow-up and similar cases in the family.
Current medications
The list with names and doses, with special attention to anticoagulants and antiplatelet agents — they change the planning of any procedure.
Recent laboratory tests
If available: complete blood count, coagulation panel and other blood tests requested during the investigation.

What a second opinion is not
It is not a remote diagnosis. No diagnostic conclusion is reached solely on the basis of files sent over the internet.
It does not replace an in-person consultation. The neurological examination and direct clinical assessment remain indispensable for any treatment indication.
It does not break your relationship with your attending physician. A second opinion exists to add information to the decision, not to transfer your follow-up care.
It is not an emergency channel. In the event of a sudden and intense headache, loss of strength, or changes in speech, vision or consciousness, seek an emergency room immediately or call 192 (SAMU, in Brazil).
There is no promise of results. No analysis guarantees an outcome, a cure or the absence of complications — what exists is clear information to help you decide better.

Common questions
Do I need to have already had imaging studies?
Yes. A second opinion is a review of exams that have already been performed. Without imaging, there is no material to analyze — in that case, the path is an evaluation consultation, in which the investigation is requested.
Can I request it on behalf of someone else?
Yes. Relatives and caregivers often initiate contact. Please state your relationship to the patient and the patient's name — the clinical material is protected by medical confidentiality and is discussed only with the patient or with whomever they authorize.
My exams were done in another city or country. Does that work?
Yes. The analysis is based on the digital files of the exams, regardless of where they were performed. Reports in another language may be sent in the original.
How is my data handled?
Exams, reports and clinical information are protected by medical confidentiality and handled in accordance with the Brazilian General Data Protection Law (LGPD). The material is used exclusively for the analysis of your case.
Is there a cost for the second opinion?
The terms of care, available formats and fees are provided by my team at the first contact, before any material is sent.
How long until I get a response?
Confirmation of receipt is sent by my team as soon as the request arrives. The time for the analysis itself depends on the volume and type of material sent, and is communicated to you together with that confirmation.
